BY PARTICIPATING IN THE INTERVIEW AND ANY FOLLOW-UP THERETO WITH ODYSSEYS INTO WELLBEING YOU AGREE TO THE FOLLOWING TERMS AND CONDITIONS. PLEASE READ THEM CAREFULLY.
1. By participating in the Odysseys Into Wellbeing interview initiative and any follow-up thereof, you expressly agree that, to the extent any answers to any of the questions asked and any information provided by you are deemed to be Protected Health Information or Individually Identifiable Health Information as defined in the Health Insurance Portability and Accountability Act of 1996, Odysseys Into Wellbeing and its owners, employees and agents are authorized to have access to such information. Odysseys into Wellbeing will keep your information private, and will not share it with any third party unless compelled to do so by law or unless separately authorized by you.
2. The role of Odysseys Into Wellbeing and its owners, employees and agents is not to provide health care, medical or nutrition therapy services; or to diagnose, treat or cure any disease, condition or other physical or mental ailment of the human body. Neither Odysseys Into Wellbeing nor its owners, employees or agents are acting in the capacity of a doctor, licensed dietician-nutritionist, psychologist or other licensed or registered professional, and that any advice or information given by Odysseys Into Wellbeing and/or its owners, employees and agents is not meant to take the place of advice by such professionals. If you are under the care of a health care professional or currently use prescription medications, you should discuss any dietary changes or potential dietary supplements use with your doctor, and should not discontinue any prescription medications without first consulting your doctor.
